The Certificate of Need (CON) application has been updated and is now available online in pdf format. The application is also available in Word format upon request. Effective immediately, this application must be used for all CON proposals that are subject to review. To access the application, please go to the Authority’s website

In addition, application support documents are also available on the Authority’s website under the ‘Certificate of Need’ tab. The Authority will no longer mail out paper copies of the CON application or support documents unless requested by the applicant.
